All patients are provided with a home healthcare call button for the first 60 days of care. With a push of a button, our patients are connected with call representative that will assist them in getting their needs filled 24/7/365. The SE Connect device also detects falls, providing peace of mind for both the patients and their loved ones.

SE has created a dedicated cardiac program, including heart failure disease management. Providing support and education to patients with heart-related issues helps patients avoid trips to the emergency room and hospital stays.

SE’s advanced Remote Patient monitoring Program allows patients to have their blood pressure, heart rate, weight, and blood oxygen levels remotely monitored by a healthcare professional. SE is the only home health provider in the region to utilize the spirometer for telemonitoring of COPD patients. Ease of use, alerts, real-time monitoring of vital signs, and live video conferencing are just a few of the many benefits of this technology.

For patients being treated for cancer, SE has the ability to deliver care and treatment options in the comfort of the patient’s home. The Oncology Certified and trained team is ready to provide comfort and care to patients with cancer.

On Day 1 with SE, the rehabilitation team begins providing care to patients with orthopedic, neurological, and a variety of post-operative conditions. From the start, the team of therapists assess the total patient to determine the best course of treatment to get them back to the active life they deserve.

SE provides skilled infusion therapy to patients requiring high-tech nursing care in the home. Specially trained nurses have experience in a wide variety of venous access devices and intravenous medications. These capable nurses will administer medications in the comfort of the patient’s home and teach the patient and family to manage care for the best possible outcomes.

SE Behavioral Health Program team members use a detailed assessment and develop therapeutic interventions designed to improve outcomes, along with an educational component provided to patients and caregivers related to anxiety disorders, depression, and adjustment disorders.
