About SE Home Health Care

Providing in home health care is a primary goal of Southeastern Health Care at Home (SE). Its tradition and roots are grounded in bringing clinically-based, compassionate health care into patients’ homes.

Over time, the advancement of technology and clinical structures has allowed SE to expand its service offering. Patients can receive home-based care for illnesses such as cancer, cardiopulmonary conditions, and wound care, among others. SE’s clinicians can also provide individualized care plans for patients facing memory loss, allowing them to age in place.

The clinical teams are physician-led, ensuring that all patients receive the best care possible in the setting that is most comfortable to them. The caregiving teams are recruited to be able to provide care that is not only clinically appropriate, but delivered in a caring and compassionate manner that respects the home setting of the patient.
