Cardiopulmonary

Southeastern Health Care at Home’s (SE) cardiopulmonary care includes Cardio Care and cardiac rehabilitation. SE’s physician-led clinical services work with patients, caregivers and providers to develop a plan of care that can be delivered in the home while maintaining the level of care necessary to ensure that patients are successful in their treatment.
The Cardio Care Program is a home-based Heart Failure Disease Management program that is focused on:

  • self-management
  • medication management
  • physician follow up.

The home-based cardiopulmonary rehabilitation combines clinical care, daily exercise programs and treatment, and Remote Patient Monitoring. Cardiac rehabilitation services focus on one or more of the following:

  • Heart Failure
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Transitional Care after surgery or cardiac event.

All cardiopulmonary home health services are delivered by SE professionals who ensure the clinical integrity of the services, while providing the care and compassion that make a difference to patients and caregivers.
