CardioCare: Heart Failure Management

Southeastern Health Care at Home’s (SE) CardioCare Program provides assessment and education to empower patients to manage their own chronic diseases. Heart Failure Disease Management emphasizes three core principles:

  • Self-management
  • Medication management
  • Physician follow-up

The program follows an evidence-based, clinical protocol that is designed by physicians to avoid emergent transfers to the hospital and improve quality of life. The program includes patient and family education for healthy lifestyle choices and self-monitoring, and integrates modern technology, including Remote Patient Monitoring.

High-tech infusion therapy by skilled clinicians is available when needed. Milrinone (cardiac inotropes) and IV Lasix can be provided in the home.
