About SE Hospice Care

Southeastern Health Care at Home (SE) Hospice Care Services allow patients and loved ones the freedom to spend meaningful time together in a comforting environment. With the support of the hospice team, patients can avoid spending precious time in a physician’s waiting room or making unwelcome trips to the emergency department.

Is Hospice Care the Right Choice?

Patients with life-limiting illnesses who are not candidates for, or who do not wish to continue, with curative treatments, may decide that hospice care is right for them. Patients or caregivers who have questions about hospice or feel that hospice care is the best choice may contact SE for more information. Our Hospice team will consult with patients, caregivers, and physicians. When appropriate, the physician can write an order for hospice care.

Insurance Coverage

Hospice is a covered benefit for Medicare part A and for Medicaid recipients. Many private insurers offer some type of hospice coverage as well.

Patients who select SE Hospice Services will receive home visits from hospice team members. Medical equipment, supplies, and medications that are related to the terminal illness and part of the specialized care plan will be delivered to the patient and caregiver.

The care plan may also include Home Health Aide Services to assist with personal care and volunteer services if requested. Patients, families, and caregivers will also have access to a medical professional 24/7 by calling 833-888-5165.
