Southeastern Home Health Supportive Care Program
Supportive Care is a specialty program for Southeastern Health Care at Home patients. The program is a holistic interdisciplinary approach to patient care that aims to improve quality of life for Southeastern patients by providing the right care at the right time through the provision of supportive services that target patient-specific goals. The program integrates treatments and symptom management provided by the home health team with specialty care for more complex needs provided by the interdisciplinary team. Supportive Care is an extra layer of support for the Home Health patient.
In addition to the Home Health RN and team providing treatments and symptom management, patients with supportive care will have:
- An assigned Social Worker to assist with assessments tools and goal setting
- The consultation of and Interdisciplinary Team
- Palliative Team contacts PCP for Palliative Consult
